ERP (Exposure and Response Prevention)

Exposure and response prevention, or ERP, is a specialized form of exposure therapy and the gold-standard treatment for OCD. You make planned contact with what triggers the obsession while deliberately not performing the compulsion that usually follows, which teaches the brain that the anxiety passes on its own.

The response-prevention half is what distinguishes ERP from general exposure. In OCD, the compulsion is what keeps the loop alive: it relieves the anxiety just enough to confirm the obsession mattered. Blocking the ritual, with support and by agreement, lets the anxiety rise, crest, and fall without it, and each pass weakens the loop.

ERP is structured and gradual, beginning with triggers you rate as manageable. It is demanding work, and it has some of the strongest outcome evidence in mental health care, which is why it anchors serious OCD treatment.

Common questions

How is ERP different from regular exposure therapy?

Regular exposure targets feared situations. ERP adds the second, essential half for OCD: preventing the compulsive response that normally neutralizes the anxiety, so the learning actually sticks.

Does ERP work for mental compulsions?

Yes. Reviewing, reassurance-seeking, and mental checking are compulsions too, and ERP addresses them the same way: contact with the trigger, then practicing not doing the ritual, including the invisible ones.
